Heritage is the leading Homecare agency in New Mexico. We are currently looking for a vibrant, outstanding Physical Therapist who is currently licensed in the state as a Physical Therapist to join our Albuquerque Team on a PRN basis. The PT will work with our close knit team, whom in collaboration with a multi-disciplinary team develops a plan of treatment for each physical therapy patient and provides skilled physical therapy services in accordance with physician orders.


POSITION SUMMARY:
An individual who is currently licensed in the state as a Physical Therapist in collaboration with multi-disciplinary team develops a plan of treatment for each physical therapy patient and provides skilled physical therapy services in accordance with physician orders.


JOB DUTIES

1. Provides skilled physical therapy services/interventions in accordance with physician"s orders and plan of care.
2. Assess patient needs initially and ongoing to determine level of functioning including OASIS assessment at appropriate times.
3. Consults and coordinates with physician and interdisciplinary team to develop plan of care and evaluate team goals for each patient.
4. Establishes therapy goals based on assessment and physician orders.
5. Instructs patients/family/caregivers on patient"s total physical therapy program.
6. Advises, consults and instructs members of the multi-disciplinary team in specific physical therapy modalities.
7. Prepares and submits timely and accurate OASIS, clinical and progress notes, payroll and other required agency documentation per agency policies and procedures.
8. Coordinates services in consultation with other home health disciplines.
9. Adheres to all agency policy and procedures while providing services to agency patients.
10. Observes and reports patient progress and/or changes to physician and appropriate disciplinary team members.
11. Participates in and attends multi-disciplinary team conferences and in-services as scheduled.
12. Evaluates and provides supervision of Physical Therapy Assistants and Home Health Aides as appropriate
13. Evaluates effectiveness and outcomes of care; modifies interventions to meet desired outcomes.
14. Participates in discharge planning of patient from home healthcare.
15. Performs other duties as assigned.

Skills / Requirements

Qualified candidates will meet the following criteria

Education:
Graduate from a school of physical therapy accredited jointly by the American Physical Therapy Association and Council on Medical Education of the American Medical Associations.

Experience:
Minimum of two years experience as a physical therapist in an acute care facility, outpatient setting, community health service or home health agency in the past three years.

Licensure/Certification:
Current state license as a Physical Therapist. Current CPR. Valid drivers license. Current auto insurance. Current health certificate.

Other Qualifications: Ability to travel in service area.
