Job Description:

POSITION SUMMARY
Responsible for coordinating client cases in the Services @ Home division in which a wide range of support services are delivered to help clients in their own homes. Responsible to meet staffing level goals each month. Activities may include planning client needs, scheduling staff to meet client needs, and assisting with hiring, orienting and training staff to provide services. Provide on-call service after hours, on weekends and holidays to support staff and clients in meeting service needs.

  • Basic knowledge of disabilities home healthcare and federal/state benefits for people with disabilities such as Medicaid, Social Security and Waiver programs.

JOB DUTIES:

  1. Provide excellent customer service to internal and external customers.
  2. Working with the computer for 8 hours or more per day.? Organize Services @ Home Division client cases and staff timely with qualified caregivers.
  3. Update and coordinate client and staff schedules to reflect cancellations, rescheduling, logging, addition, or modification in the automated scheduling system.
  4. Responsible for reaching established staffing reliability goals.
  5. Assist in planning, developing, organizing and evaluating the services being provided to clients.
  6. Monitor, evaluate, and report client information to the appropriate personnel.
  7. Update Individual Plan of Care documents as requested by Operations Supervisor.
  8. Take incoming phone calls from clients and caregivers 4 to 8 hours per day.
  9. Provide On-call service after hours, on weekends and holidays to support staff and clients in meeting service needs.
  10. Provide in-home care in needed situations.
  11. Participate with the payroll process as it relates to scheduling issues to ensure timely and accurate payroll and billing.
  12. Provide ongoing guidance of caregivers to ensure that all requirements are met.
  13. Provide accurate and timely reports to the Scheduling Operations supervisor.
  14. Market Heritage to the community to increase sales and promote employment opportunities.
  15. Responsible for reporting and resolving employee/client incidents and complaints.
  16. Participate in Quality Improvement teams and activities as requested, including random employee record audits.
  17. Provide accurate and timely reports to keep manager informed regarding operation.
  18. Perform other duties as assigned.
