 Requirements:<\>

 

Level I:<\>

 Masters degree in Social Work required and must hold current license from the State of New Mexico (LMSW). One to two years hospital social work experience, protective services, or practicum in acute care settings experience desired. National Case Management certification preferred

Level II:<\>

Masters degree in Social Work required and must hold current license from the State of New Mexico as a Licensed Independent Social Worker or Licensed Medical Social Work license with five years hospital social work or discharge planning experience in relevant setting. One to two years hospital social work experience, protective services, or practicum in acute care settings experience desired. National Case Management certification preferred

Level III:<\>

Masters degree in Social Work required and must hold current license from the State of New Mexico as a Licensed Independent Social Worker plus five years hospital social work or discharge planning experience in relevant setting. Licensed Medical Social Work licensure plus ten years hospital social work or discharge planning experience in relevant setting. Working knowledge/experience in case management. National Case Management certification preferred

 

Summary: 
<\>Responsible for evaluating, developing, and implementing appropriate discharge plans which take into account the psychosocial and financial needs of patients and their families and functioning as a key member of the interdisciplinary healthcare team. This is directed at supporting and maximizing the social functioning and adjustment of the patient and their family


Preferred Qualifications:

Responsibilities: 
<\>*The medical social worker is responsible for the identification and assessment of psychosocial problems and will work with patients and families achieve resolution or will provide referrals to the appropriate agencies for the service.

*The medical social worker shall be available for patients and their families through many aspects of care. This includes psychosocial problem-solving activities in admission and discharge phases and assessment of financial needs and available resources, which contribute to a safe and appropriate discharge to the community setting.

*Results of assessments and interventions are documented in the medical record.

*The medical social worker functions as an advocate and liaison to the multidisciplinary healthcare team and is responsible for discharge plans, which will achieve optimal patient outcomes.

*The medical social worker collaborates as a key member of the multidisciplinary healthcare team. Collaboration includes the active participation of the social worker in the team decision-making process. He/she is responsible for helping the team members understand function and role of the social worker in the clinical setting.


*The medical social worker acts as an expert resource and contact for the multidisciplinary team, including other social workers or community case managers, physicians, ancillary staff and nursing staff on a multi-state and regional basis to help promote the optimal continuity of care for the patient in discharge or transfer arrangements.


*Facilitates effective communication between administration, staff, physicians, other department, patients, families and the general public. Participates in conflict resolution as necessary and acts as an effective problem solver.

*Exemplifies a high standard of leadership qualities and professionalism. Mentors the staff in developing and achieving professional goals.

*Maintains current knowledge of information in related field through seminars, workshops and publications.

*Assures implementation of change projects/ initiatives.

*Assures that the population's psychosocial and discharge planning are addressed and fulfilled.

*Participates in all policies, records, data and reports as necessary for satisfaction of State, Federal, and Joint Commissions regulations.

*Serves as an educational resource regarding the functions of department specific programs to both colleagues and customers.


*Assessments

*Completes MDS, RAPS, Care Plans

*Performs Utilization Review

*Performs other functions as required.
